Checklist

Before you send your application form to HMCTS Probate you will need to complete this checklist of your enclosures.

    This form (PA1P) where a person who has died has left a Will.
    The last original Will and any codicils made since that Will.
    An offical copy of the death certificate of the person who has died.

    As well as the application fee, there is a fee for each official copy of the Grant of Representation.
